The film opens with a montage of women through different decades, as seen from the eyes of the same unseen person. They each appear happy at first, but then grow upset and weary with the person. The montage ends with the women screaming and covered in blood.

In the present day, a couple – Liz (Tatiana Maslany) and Malcolm (Rossif Sutherland) – go to a cabin in the countryside for a romantic weekend getaway. Liz calls her friend Maggie (Tess Degenstein) to talk about the cabin and the state of her relationship with Malcolm, sounding rather unenthused. Liz later finds a cake that Malcolm claims is from the housekeeper.

Liz and Malcolm try to have a private dinner until Malcolm’s cousin Darren (Birkett Turton) arrives with his date, Minka (Eden Von Weiss). Darren is rude to Liz and claims that Minka cannot speak English. When the two women are left alone, Minka comments that the cake “tastes like shit”. When Liz and Malcolm are alone again, Liz tries the cake and confirms that it tastes like shit.

Liz starts to see weird visions of the women from the opening montage, as well as a vision of what appears to be someone holding a rifle to the back of her head. She wakes up and goes to the kitchen, where she starts eating the cake. There appear to be bloody fingers inside, but Liz ravenously eats it.

The next morning, Minka is seen walking through the woods alone. She hears a noise and goes near a tree, where an unseen force pulls her upward and kills her.

Malcolm tells Liz that he needs to go into town to deal with a patient of his, leaving her home alone. She has another conversation with Maggie, who implies that Liz is a “side piece” and makes her upset. After hanging up, Liz sees what looks like a woman’s body in the woods before seeing Minka and a smaller version of her in the kitchen. Minka tries to give her a cryptic warning about “the children” thinking Liz is their mother.

Darren shows up at the door, demanding to be let in. After doing so, Liz hides in the bathroom, while Darren grabs a meat cleaver and walks around the house, ranting drunkenly. Soon, he gets killed by unseen forces and has his corpse dragged away. Liz starts to get more freaked out and calls Maggie to come get her, but the signal keeps cutting out.

Malcolm returns home that night, and Liz starts to express her fears to him. He tries to calm her down, and they drink some wine. Liz gets more suspicious and locks herself in the bathroom, finding what looks like a picture of Malcolm’s wife and daughters. He opens the door since the lock doesn’t really work, and she starts to accuse him of cheating, but Malcolm sits Liz down to tell her the truth. 200 years ago, he and Darren were kids who found a pregnant woman that resembled Liz on their family’s property, and they shot her in the leg. They took her back to their pigpen, where she gave birth to the creatures that Liz has been seeing. They shot the woman in the head, and have been bringing the creatures women for them to feed off of, which has granted the men longer life and youth. The cake was meant to drug Liz the way that Darren did with Minka. Liz tries to escape by climbing out the window, but she falls in the lake.

Malcolm recovers Liz and locks her in the basement. There, the creatures present themselves to Liz and have multiple eyes, mouths, and other appendages. They embrace Liz due to her looking like their mother, whose head they have kept preserved in a jar of honey. Upstairs, Malcolm gets attacked by the creatures.

In the morning, Malcolm wakes up, now aged horribly and hanging upside down. Liz approaches him, with her eyes looking black and cloudy like the creatures. She forces him to eat some of the cake. Malcolm tells Liz he loves her, but she tells him he’s lying. She then drowns Malcolm by dunking his head in a jar of honey before walking away.

A couple named Liz and Malcolm go for a romantic weekend getaway at Malcolm's family cabin. There, Liz starts to encounter weird visions as she questions her relationship with Malcolm.

200 years earlier, Malcolm and his cousin Darren were kids when they shot a pregnant woman on their family's property who resembled Liz. They brought her back to their pigpen, where the woman gave birth to creatures. After killing the woman, Malcolm and Darren have lured women for the last two centuries to the creatures for them to feed off of, which has kept them young and given them a longer life. Darren is killed by one of the creatures after luring his latest girlfriend Minka to them. Liz tries to escape but is caught by Malcolm.

The creatures embrace Liz as one of their own due to her resemblance to their mother. They work together to age Malcolm rapidly before drowning him in honey.
