The film opens on a plane, where dozens of assassins are duking it out in the middle of other horrified passengers. One of the assassins shoots through the window, causing it to break open and depressurize, sending some people flying out while the plane starts to take a dive.

Twelve hours earlier, CIA Agent Aaron Hunter (Julian Kostov) learns that a mysterious terrorist known as “The Ghost” has taken down one of their covert operations in Bangkok. Despite Hunter wanting to keep this a secret, Director Katherine Brunt (Katee Sackhoff) finds out and declares The Ghost to be a threat to national security.

Brunt contacts Lucas Reyes (Josh Hartnett), a disgraced former agent who has been forced to stay in Bangkok since Brunt had him put on the no-fly list for the last two years. Lucas just ignores her out of spite, but she still calls the bar that he spends his days drinking at. He gets beaten up by some thugs before giving up and talking to Brunt. She wants him to find Ghost and bring them into custody in San Francisco. Despite Lucas’s hatred for Brunt, he ends up taking a taxi to the airport as she instructed.

Lucas gets a passport from one of Brunt’s guys and boards the plane later. As the plane is already in the air, Hunter and Brunt learn from another agent that there is a bounty for $10 million on Ghost’s head, making them realize that the plane is full of killers. Onboard, Lucas starts talking to the passenger next to him, Cayenne (Marko Zaror), but he starts to feel dizzy. Cayenne brings Lucas into the bathroom, where it’s revealed Cayenne is one of the killers, having drugged Lucas with xylazine thinking he is out for the bounty as well. Lucas nearly gets his neck broken until he manages to use his strength to push Cayenne upwards, pushing the ceiling sprinkler through his head. Lucas makes himself vomit the rest of the drug before texting Brunt about killing someone, which puts her in a panic.

Flight attendants Isha (Charithra Chandran) and Royce (Danny Ashok) hear the commotion and order Lucas to come out before the Sky Marshal tries to apprehend him, but Lucas knocks him out. After explaining his situation to the flight attendants, they get him a change of clothes and help hide the bodies. Royce later finds that the real Sky Marshal was killed and dumped into the cargo hold. When Isha takes him to the cargo hold, Lucas quickly figures out she is The Ghost because she’s experiencing cold sweats and a wound opening up. They form a reluctant alliance knowing that the plane is full of people out to kill her. One assassin overhears their conversation and sneaks out.

Other assassins start to make their moves. Isha is attacked by a Sikh man who lost millions in cryptocurrency thanks to The Ghost, but she had already poisoned his coffee in anticipation. Lucas gets attacked by a female assassin who gets killed when he jams a broken wine glass handle into her eye. Three other assassins from a group called Thai Wan Deng also attack, but Lucas kills them all before he finds his picture on one of their phones. Isha insists that she has no idea why the killers are after him as well.

After Brunt finds out that killers are after Lucas, she comes clean to Hunter and says that Lucas was a former Secret Service agent who lost it all after murdering a diplomat that liked to assault young prostitutes. The man was the brother of the CIA’s Deputy Director, and since Lucas and Brunt were also in a relationship at the time, she sold him out to the agency to save her skin and boost her career, which is why Lucas despises her now.

Soon, every last killer onboard the plane gets Lucas’s picture on their phone along with the bounty on him. Lucas goes out and starts to fight them all, leading to a chaotic fight in economy class. Isha is aided by a woman called Master Lian (JuJu Chan Szeto) and her two companions, as Isha helped take back her land. Most of the assassins are killed off. Lucas is injured but has his wounds tended to. Brunt soon learns who The Ghost really is.

Lucas goes down to the cargo hold with Isha, where he finds that she has brought a large device onboard. She explains her backstory, where she was trafficked along with other children, and when her captors discovered her technological capabilities, they forced her to commit various atrocities across the globe (involving drones, hacking major systems, etc). Isha escaped and went off the grid, and she built the device to gather information on various criminal networks. Lucas weighs the option of turning Isha in and being able to go home again. Meanwhile, it turns out that Hunter knew about the device from a close source and orchestrated having all the hitmen go after both Lucas and Isha so that they can all kill each other and get their hands on the device to make sure their own dirty secrets aren’t exposed.

With the other assassins closing in on them, Lucas takes what he thinks is adrenaline (actually something that makes him trip balls) before Isha and Master Lian’s clan go after them in a bloody melee. Lucas uses a pickaxe against one assassin and rains blood on the other passengers. One of the female assassins gets shot in the head before the window gets broken open and starts to suck some people out. The pilots manage to dive the plane to a more stable altitude, but they are both killed moments later by two other assassins. On the ground, Brunt and Hunter are waiting for the plane to come down, but when it doesn’t, Brunt executes Hunter with a bullet to the back of the head. Isha tosses Lucas a chainsaw so he can mow down the rest of the killers.

The other flight attendants gather the passengers to safety while Lucas and Isha go to the cockpit to fly the plane away from San Francisco. Brunt contacts Lucas, but he denies having seen the device and he tells Brunt that he will not be bringing Isha to her, leaving her to deal with her messes on her own. Lucas and Isha thank each other for saving one another before Lucas passes out.

Lucas wakes up in a hospital, where he finds that Isha has brought him to rescue kidnapped children in the middle of a war zone. He can only respond with, “Oh, fuck!”

Disgraced Secret Service agent Lucas Reyes is tasked by CIA Director Katherine Brunt to locate a terrorist known as "The Ghost" and bring them to San Francisco for prosecution. Onboard the plane, Lucas finds that plenty of other assassins are out for The Ghost due to a $10 million bounty, as well as one on Lucas.

The Ghost is revealed to be a flight attendant named Isha, a former trafficked child who was forced to commit atrocities by her captors after learning was technologically talented. After going off the grid, Isha developed a device that has information on various criminal networks, including the dirty dealings inside the CIA. One of the agents, Aaron Hunter, orchestrated the assassins going after both Lucas and Isha so that they can all kill each other and he and Brunt can get their hands on the device. When the plane fails to land as expected, Brunt executes Hunter.

Lucas and Isha team up alongside a trio of female assassins to take on all of the killers on the plane. After all of them are dead, Lucas makes it clear to Brunt that he is not bringing Isha to her, and he lies about seeing the device, implying he may expose Brunt's crimes. Lucas is then forced to join Isha on a dangerous mission to rescue other trafficked children.
