Anime recap for context: Denji is a teenager living in Tokyo, broke and down on his luck until he is fused with his Devil-dog, Pochita, to become an immortal half-Devil being known as Chainsaw Man, as he can produce chainsaw from his arms and make his head into a giant chainsaw. Denji begins to work for the Public Safety Devil Hunters in Division 4, led by Makima, whom Denji has a crush on. Denji works and lives alongside fellow Devil Hunters, Aki Hayakawa and Power, another half-Devil, or “Fiend”, with the power to manipulate blood. After Denji/Chainsaw Man’s battle with Katana Man, Makima has employed three other half-Devils – Beam/Shark Field, Violence Fiend, and Angel Devil – to aid Division 4 in their endeavors.

The film starts with Denji describing a weird dream that he has been experiencing, with Pochita telling him to avoid opening a mysterious door that he keeps coming across. When Denji wakes up, he tries talking about his dreams to Power, but she ends up punching him so hard that he flies up, as a result of her having recently increased strength.

Makima sends Power to be drained of excess blood, then assigns Beam to be Denji’s partner until Power returns. Beam is overzealous to work with Denji, something that freaks him out until he learns that Beam will do anything that he tells him to do.

Makima later invites Denji on a date to the movies all day. They do not enjoy the first batch of films, but the last film is one that Makima has already seen. Both Denji and Makima tear up during the end of the film. Makima assures Denji that he has a heart, which invigorates him.

Feeling empowered, Denji starts to do good deeds, such as donating to victims of Devil attacks. He also starts to contemplate asking Makima to officially be his girlfriend. Denji gets caught in a storm and hides out in a phone booth, where he is joined by another teenage girl named Reze. Denji performs a “trick” where he regurgitates a flower that he ate earlier, which seems to amuse her. Reze invites him to join her later at the nearby cafe where she works.

Denji quickly appears at the cafe for lunch. Reze sits down next to him and seems to start flirting with him. Although Denji finds himself attracted to Reze, he becomes conflicted since he considers himself faithful to Makima.

Meanwhile, Aki has been partnered up with Angel on their patrols to prepare for another confrontation with Gun Devil. They do not get along, as Angel’s half-Devil side has made him indifferent to the feelings and suffering of humanity, because he thinks those things are necessary. He refuses a human’s plea for death after a Devil attack, forcing Aki to do the deed himself. Makima tells Aki to just use Angel for his own benefits.

An assassin known as “Mohawk Man” is shown to be working alongside Typhoon Devil, as they and various other Devils are out for Chainsaw Man’s heart. Mohawk Man proceeds to take out a family before going on his mission.

Denji starts to spend more time with Reze and acknowledges that he likes her. She invites him to break into a school late at night, where Denji admits to Reze that he did not go to high school. They go skinny dipping in the swimming pool, further fueling his attraction to her. Later, Mohawk Man ambushes Reze in the halls while Denji is in a classroom. He chases her to the roof, where she pretends to be distressed, only to reveal her own skills by jumping onto Mohawk Man and strangling him to death. Reze then calls upon Typhoon and re-establishes their previous contract, since Typhoon is aware of how powerful Reze really is.

Reze later invites Denji to go check out fireworks with him. She tries to convince him to leave Public Safety and run away with her, but she declines. Reze pulls him in for a kiss but ends up biting off his tongue and cutting his throat. Before she can kill Denji, Beam emerges and saves him. He had detected Reze’s scent and realized that she is the Hybrid known as the Bomb Devil. Like Denji, she has a string that she pulls to turn her body into a bomb-like material that allows her to create random explosions. Beam and a couple of passing Devil Hunters try to fight Reze but are easily beaten.

Beam takes a weakened Denji to a Division 2 hideout for backup, as Aki and Angel are there as well. Reze arrives and proceeds to slaughter most of the Hunters, while Aki, Angel, and Beam drive away with Denji. Violence and cowardly Devil Hunter Kobeni create a brief distraction for Reze, but it only stalls her for a moment. Reze pursues them through the streets, causing more explosions and killing innocent people. Denji wakes up and becomes Chainsaw Man to battle Reze, now out of spite for her betrayal. Denji’s chainsaws prove to be no match for Reze’s bomb attacks, as she is practically invulnerable to attacks. She continues to literally blow up in Denji’s face until he is too weak to regenerate from his wounds.

Aki and Angel intervene in the fight, but Typhoon steps in to create another problem for the Hunters. Angel uses the blood of a dying woman to revive Denji. In the midst of the chaos, Angel prepares to die, but Aki saves him. However, Aki making physical contact with Angel shortens his life expectancy by two months. Knowing that his chainsaws alone will not stop Reze, Denji decides to “work around” it by using the loose chains on his arms. He then rides Beam in his Shark Fiend form into battle against Typhoon, slicing the Devil up into a gigantic blood tornado.

Beam goes after Reze but gets severely wounded before she tries to get Denji again. Their fight takes them towards the water. He uses the loose chains to wrap around Reze’s body and pull her underwater, which prevents her from being able to explode.

Reze wakes up fully revived on a beach, as Denji rescued her and made sure she came back. Beam also washes up along with them both. She and Denji sit together, with her saying that she failed her mission and that she never really had feelings for him. Denji suggests to take her up on her offer to run away together. Reze appears to go and try to kiss Denji again, but she just snaps his neck so he can’t move. She starts to leave, but he tells her he will wait for her at the cafe.

Public Safety Captain Kishibe talks to Aki about Reze. She was experimented on as a child by the Soviets, and she was sent under them as a spy to go after Denji.

Reze is at the train station, ready to flee for good, until she appears to have a change of heart. She starts running back to the cafe, but she is stopped by Makima in an alley. She talks to Reze for a bit but catches as she attempts to detonate herself, and Makima uses her abilities to sever Reze’s arm. As she tries to keep going, Angel impales Reze with a spear. As she lays dying, she looks over to the cafe, where Denji is waiting for her. Angel then goes to a rooftop and asks a mouse is the city is okay.

After the credits, Denji is still waiting at the cafe at the end of the day, until Reze’s boss lets him know she is gone for good. Power then bursts in through the door, triumphantly returning. She sees Denji with a bouquet of flowers and assumes they must be for her. Denji considers giving them to her, but he starts to eat the bouquet out of pettiness. He and Power start to bicker over it, while the sound of a clanging behind the forbidden door can be heard.

Denji is a teenager in Tokyo who works as a Devil Hunter for Public Safety with his friends Makima, Aki, and Power. Denji can transform into the human-Devil hybrid known as Chainsaw Man, who produces chainsaws from his arms and head. Although Denji is in love with Makima, his feelings become complicated when he meets another girl named Reze, who appears to have taken a liking to Denji.

Reze is really the hybrid known as Bomb Devil, as she was sent by Soviets to go after Denji/Chainsaw Man's heart. Denji is aided by Beam/Shark Fiend, plus Aki and Angel Devil, in the fight as Reze causes destruction all across Tokyo. After Denji and Beam kill her accomplice, Typhoon Devil, Denji uses his loose chains to wrap around Reze and throw her underwater with him so she cannot explode.

Denji lets Reze run away instead of allowing Public Safety to catch her. However, she seems to have a change of heart and runs back to Denji, only for her to get ambushed and killed by Makima and Angel.
