The movie starts at the Olduavi research base in one of the canals of Mars. We go inside and see a bunch of scientists running from something that is pursuing them relentlessly. One by one, the scientists all get taken (one guy gets pulled through the floor) until it's just a woman and the head scientist, Dr. Carmack (Robert Russel). Carmack goes into a room -- his office -- and shuts the door. The woman tries to get through, but Carmack won't let her and the door closes on her wrist, cutting her hand off. She gets dragged away by the...whatever...while Carmack frantically makes an emergency SOS call just before the monster breaks through and attacks him.

In California, at a USMC base, we see a man we know only as Sarge (The Rock) getting orders from his superior that are instructing him to take his team up to the base on Mars, recover as much data as possible, save as many as he can, and if necessary, do a search and destroy. His response shows that he's a mission man who will go to any lengths to complete the mission. Elsewhere in the base, we see the seven members of Sarge's crew -- religious man Goat (Ben Daniels), big quiet guy Destroyer (DeObia Oparei), cocky ace Duke (Raz Adoti), wisecracking hick Portman (Richard Brake), Chinese immigrant Mac (Yao Chin), rookie soldier The Kid (Al Weaver), and jaded vet John Grimm (Karl Urban), whose official call sign is "Reaper." Sarge comes in and tells them that their leave is officially cancelled and lets them know about the mission. Most of them head upstairs, but Sarge stops Grimm and cautions him to let this one pass before he goes to brief the others.

Outside, we see the squad loading into their chopper, checking their weapons, and teasing Kid when Grimm pops in and full armor, determined to come. Sarge asks him if he's sure and Grimm nods as the chopper takes off to transport them to The Ark, a big transport facility in Nevada. When they arrive a large elevator comes up from underground and Duke asks Kid if he had a big breakfast. They go into the facility and are greeted by a doctor who cautions them not to stand to close to the center. Sarge warns the doctor that there may be a quarantine put into effect on Olduavi, so they'll have to be ready for that. The doctor agrees and presses a button, which causes a gelatinous substance to start floating out of a narrow tube in the floor. A countdown begins, Sarge steps forward, and--

--we see the members of the team finishing throwing up their breakfasts at the Olduavi research base. Sarge yells "Game time" and the men are greeted by some of the doctors in the main facility, including Grimm's sister Samantha (Rosamund Pike) and one of the workers, Abraham Pinkowski (Dexter Fletcher), a guy with some motorized wheel system instead of legs and a frightened guy attitude who says to our boys, "Call me Pinky." Duke starts making eyes for Samantha as Pinky pulls up a map of the facility and hooks their video-audio feed into his system. Sarge tells Mac to stay with Pinky and guard the entrance to the arc while him and the rest go into the facility through a bio-gate, which is basically a big shimmery wall that can close and you can walk through...like water without the wetness. They fan out and after leaving Grimm with Samantha so that she can download some information the facility was working on (they mapped the twenty-fourth chromosome according to what Samantha tells John), the other six spread out in pairs to cover the facility.

Once inside the facility, everyone splits up -- Portman and Kid, Sarge and Destroyer, Duke and Goat -- and they go through the motions. At one point Sarge and Duke find an armory in which Sarge finds a new kind of weapon -- a Bio Force Gun (BFG). He tries to access it but is unable to since his fingerprints aren't in the system before moving on. Meanwhile, Portman and Kid find a room filled with animals barking loudly and Duke and Goat find a really deep cell with electrified walls. While this is going on, John and Samantha are insulting each other until Samantha lets slip about Lucy, some animal remains that the archaeological team found on the surface. We see Lucy holding her baby, and Samantha says that the twenty-fourth chromosome may have had superhuman effects on the ones who had it. At some point, Portman and Kid are looking around when they find a survivor -- Dr. Carmack, who is leaning against the wall and gazing. Everyone comes rushing for him and though he seems VERY out of it, they get him up to the medical room where he manages to get enough together to tell Samantha, "Shut it...shut the gate..." He goes unconscious, and everyone goes back into the facility, including Grimm. The exception is Duke, who stays behind to keep an eye on things. They also stop to grab Mac, who guards the exit and gives Pinky a gun and some grenades in case anything gets past them.

As they're going deeper into the facility, we see little interludes with everyone. Kid starts getting nervous thanks to some choice words by Portman, so he asks Portman for something "for the nerves", which Portman eagerly hands over two of. Grimm and Goat are on one of the upper floors and we see Goat accidentally knock a barrel marked flammable down a flight of stairs. He curses and then takes out a knife and cuts a cross into an arm already marked with tons of scars. When Grimm looks at him for an explanation, Goat says, "I took his name in vain." From there they go back into the holding room Goat and Portman were in before, only now all of the animals have apparently broken out of their cages...and some have left skin behind. They hear something behind them and see someone eating one of the animals. He sucks in a mouse tail and lunges for the boys, but they fire and take him down. Sarge asks what's happening and they tell him. Sarge tells them to leave it as he and Duke make their way down a corridor when they suddenly hear something in the vents above them. They nudge a vent open with Duke's (massive) chain gun, and he climbs up on a box, peeks inside, and kills a monkey that he thought was something else.

Meanwhile, Samantha and her assistant (who arrived at some point) are taking some blood from Dr. Carmack and Samantha finds a bunch of strange things swimming around in the blood of Dr. Carmack. When they look up from the microscope, they find to their astonishment that the late Dr. Carmack, who had just died, is gone. Meanwhile, the others are still exploring when they see something moving around. They give chase and follow it into the sewers. Destroyer guards the exit back to the facility while the others give chase. After a brief moment where we think Portman is getting attacked but he really just lost his footing, they come to a three way turn. Grimm and Kid take one Route, Sarge and Portman take another, and Goat takes the third. We see Grimm and Kid searching with Kid blathering away, and Grimm snaps and tells Kid to shut the f*** up. He also realizes that Kid's high, but before he can do something about that the monster shows up, roars, and runs off. Grimm radios Goat that it's headed for him, but Goat, whose light has gone out, radios back that he can't see anything. Grimm tells him to stay put, and the others start closing on his position. Goat thinks he hears something behind him, but nothings there. He turns back around, moves forward a little, and gets attacked by something that sticks out a tongue and attacks his neck before running off. The others arrive and see him bleeding badly, and they rush back to the medical room.

Inside the medical room, they put a fading Goat on the table and immediately start emergency resuscitation procedures, but it doesn't work -- Goat dies. Sarge pulls -- almost but not quite literally -- Samantha aside and shows her the thing they just killed in the sewers. She doesn't know what to make of it and after a moment, Sarge relinquishes. The team now heads for the dig site, intending to shut and lock all doors based on the theory that these things came from outside. Portman voices the opinion that they need to get out of there and call in reinforcements, but Sarge shouts him down. They spread out with Mac guarding the entrance, Portman and Destroyer on one side, and the other three on the other side. At one point, Grimm stops by a window and looks at the bleak martian landscape outside, recalling an accident that happened on Mars when he was younger and killed his and Samantha's parents. Sarge shakes him out of his reverie and they go on. When they arrive at the door, they notice that the scratch marks are inside the door, meaning that whatever was scratching was trying to get out, not in. At this point, we cut to Mac, who hears a noise and when he looks gets his head chopped off by a demon. Sarge and Grimm check it out and find the headless corpse of Mac (Two down, six to go...)

Back in the medical room, Samantha is doing an autopsy of the "corpse" that was brought in, and she tells Duke after removing all the organs that it's missing an appendix, which means it must have at one time been human. Suddenly they hear a crashing, and look up to see the corpse of Goat banging its head against the window while Duke and Samantha look on. Eventually the corpse hits itself in the head enough times to kill itself, and Duke runs through the bio-wall to alert the others. Samantha follows but is stopped by a dog growling at her. Duke points his gun at it when the creature suddenly runs off in time for Duke to get attacked from behind by some giant creature. It tosses him around and he and Samantha run for the lab, with her barely closing the door on the creature in time, leaving it to get stuck halfway through. Meanwhile, Portman and Destroyer are at one of the exits and Portman immodestly declares that he's going to take a shit. Destroyer just looks at him and Portman exits. He reconnoiters the bathroom and sits down. When Pinky tells Portman that he lost video feed on him, Portman says, "I'm taking a shit, you piece-of-shit cripple" and turns off his radio feed as well.

At this point, pretty much every marine is on their own. Sarge finds the hand of that lady who was "cut off" at the beginning of the film and pockets it. Grimm and Kid are off doing...something...and Destroyer for whatever reason goes to investigate the cell again. He hears something behind him and gets knocked into the cell by a demon. He blacks out for a second and comes to to find the light and chains swinging, a monitor in the cell with him, and the monster...somewhere. He looks around but doesn't see anything, so he takes the monitor and throws it against the walls, illuminating them momentarily so he can see the monster. While Pinky alerts the others that Portman is offline and all he can see for Destroyer is a bunch of wall, Portman secretly sends a broadcast to Earth requesting backup. Back with the loyal soldiers, Sarge tells Kid and Grimm to head for the last known position of Destroyer and Portman while he gets some heavier weaponry -- namely the Bio Force Gun, or as he calls it, the "Big Fucking Gun..." (...and fans of the original Doom rejoice). As Portman finishes his broadcast and comes out of the stall, he gets attacked by something hiding in the vents. Grimm and Kid hear it, and see him disappear into the ceiling as Sarge comes up with the BFG. He fires, and a huge ball of plasma spreads from it and takes out pretty much the entire corner section of the bathroom. Portman's body gets thrown to the side, and Grimm checks it, but it's just a formality -- he's gone. They also find Destroyer's dead body -- seems he was unsuccessful in his cage match and got destroyed (sorry).

With the dead bodies in tow, the remaining members of the team head for the medical bay one final time, now having cleared out pretty much all of the complex. They tell Sam everything that's happened, citing the presence of four dead doctors, two missing doctors, and one dead doctor who has gone missing, and Samantha corrects them, saying that she's done some DNA testing and the monsters are actually mutated humans. Sarge loses it and asks just what the hell they were doing here, and she shows them the research files she was downloading, which show some convict being injected with C24, some toxin they made to stimulate the twenty-fourth chromosome, and turning into one of those things. Grimm wants to destroy the data but Sarge stops him, saying it's against orders. We start to see Grimm violate protocol, but he ultimately relents. The four marines head back to the main arc chamber to secure their way home, with Sarge saying when they arrive back through the Ark, "If it breathes, kill it," while Grimm stays behind on Mars a minute while his sister shows him something else. Apparently, C24 chooses who turns into a monster and who doesn't, which means that Duke, Sarge, and Kid may be killing uninfected people. Grimm grabs Samantha and they head for the main gate, go through, and--

--arrive at the earthside facility just as the others are regrouping. Grimm tries to tell Sarge what they found, but is too late -- they've already killed everyone in the complex except for Pinky (found by Duke) and a bunch of people Kid left behind in a room somewhere. Sarge shoots Pinky and reiterates the orders, but everyone just looks at him and Kid tells him to go to hell. Sarge shoots Kid, citing mutiny, and Duke and Grimm start fighting him until a demon shows up behind Pinky, kills him, and attacks everybody else. They kill the demon just in time for a boatload of zombies to come through the door. Duke gets killed, and Sarge gets taken through a gate that Samantha and a badly wounded Grimm close. The pair limp away and hide in a small storage room. Grimm tells Samantha to get out of there and gives her a grenade that would pretty much destroy the whole complex while she injects him with a batch of C24. He gives her his pistol and says if it looks like he's turning into one of those things, kill him. She cries and nods just in time for him to go unconscious. She runs off.

I WAKE UP AND LOOK AROUND. THE FIRST THING I NOTICE IS THAT SAM IS GONE. I GET UP AND SEE A MIRROR ON THE WALL, AND THE FACE THAT LOOKS BACK AT ME IS STILL MY REGULAR OLD JOHN-GRIMM FACE. I LOAD MY MACHINE GUN AND HEAD OUT INTO THE COMPLEX, CALLING FOR SAM, BUT IMMEDIATELY FIND ONE OF THOSE DEMONS. I KILL IT AND ANOTHER ONE. I RUN INTO A FEW ZOMBIES BUT DISPATCH THEM WITH A FEW WELL-PLACED HEAD SHOTS. AS I GO UP SOME STAIRS I SEE SOMETHING ON MY NINE AND FIRE, BUT IT'S JUST MY REFLECTION. I GET TO THE TOP OF THE STAIRS AND FIND A DEMON ATTACKING ME WITH A CHAINSAW, OF ALL THINGS. I FALL BACK OUT OF ITS RANGE AND ATTACK, KILLING IT AND CLAIMING THE CHAINSAW -- COULD BE USEFUL, I DON'T KNOW. I FIND MORE ZOMBIES, BOTH DEAD AND ALIVE -- ONE OF THEM IS HANGING FROM THE CEILING WITH A MOUSE CRAWLING THROUGH IT -- AND ENTER A ROOM WHERE A HUGE PINK DEMON WITH WHEELS FOR LEGS -- PINKY, IT HAS TO BE -- RUSHES ME, TRYING TO CHEW MY LEG OFF. I GO AT IT WITH THE CHAINSAW, BUT HE'S TOO STRONG. FINALLY, I GET THE ANGLE RIGHT AND CUT OFF THE WHEELS, LEAVING THE BODY FREE TO FLOUNDER AROUND UNTIL I PUMP A FEW BULLETS INTO IT. I HEAD INTO THE MAIN ENTRANCE HALL, AND...

John looks at the chaos that surrounds him and the pile of dead bodies near the elevator to the surface. He sees Samantha and goes over to her, but just after confirming she's alive they get interrupted by Sarge, who's apparently been bitten (infected) by one of the creatures. Grimm tells Samantha to get to the elevator (which she does) and he and Sarge face off. Sarge fires the last shot from the BFG at Grimm, but Grimm gets out of the way and it just hits a wall. Sarge ducks into the main Ark Chamber and starts a game of cat-and-mouse. Grimm gets the better of him there, despite Sarge's already increased abilities, and the two agree to go hand-to-hand, no weapons. They fight, and Grimm is getting beaten pretty badly. At one point, Sarge takes a piece of metal railing and wraps it around his hand as though it's barbed wire. He keeps at Grimm and eventually, just as Sarge is beginning to really "demonize", one end of the wire goes through Grimms hand, connecting the two. Grimm notices that he's right next to the Ark controls, so he activates the Ark. Sarge starts getting sucked towards the bubble and when the countdown reaches zero, he gets pulled all the way through, with the wire ripping itself out of Grimm's hand. Grimm grabs a grenade from his belt, activates it, and tosses it through the bubble to land on Mars and destroy everything inside the complex, including Sarge. John limps back to the elevator and him and Samantha take it up to the surface, holding each other. Grimm looks at his sister, and gently whispers in her ear, "We're almost home..."
